Library Assistant
If an approved accommodation for the student involves assistance with the use of the library, the student may arrange the volunteer services through the Nexus eAccommodations online request system. Requests must be submitted five (5) full business days prior to the needed date.
Once a volunteer has been identified, the student needs to remember this point, which appears in the Contract of Responsibilities:
It is the student's responsibility to contact the appropriate person when the student needs to cancel an appointment. For example, the student needs to call his/her volunteer if canceling any appointment with the volunteer or if the student does not need a volunteer's service.
The student needs to inform the Accommodations Office if the services of any volunteer is no longer needed. The volunteer may then be available to assist another student.
Disability Related Special Parking Permits
Disability Concerns will provide Parking and Transportation Services verification of appropriate special-parking designation based on disability-related needs. No special parking permits will be issued until a parking tag is purchased. All parking accommodations must be approved by Disability Concerns and meet documentation requirements. These permits, issued for a specific length of time, allow students with appropriately documented needs to park in the faculty/staff (red tag) lots closer to class buildings than the student (green and blue tag) lots.
State Designated Accessible Parking Spaces
If the student has the State of Illinois tag or license plate permit to use accessible parking spaces (designated on campus by a blue wheelchair sign), the student should take his/her tag or vehicle registration to Parking and Transportation Services and purchase the appropriate University parking tag. No form from Disability Concerns is needed. Parking Services will forward information to Disability Concerns.
Accommodations within University Housing Services
Accommodations are available for students with disabilities or medical conditions. Accommodations that may meet the needs of students with such requirements may be, but are not limited to: accessible rooms and facilities for students with physical disabilities; accessible rooms to meet the needs of students with vision disabilities; strobe lights and doorbells with lights for students who are deaf or hard of hearing; or special rooms and facilities for medical needs.
All students requesting accommodations must have completed a contract for housing with University Housing Services. Requests for housing accommodations within University Housing for disability related reasons must be initiated by the student with a request to Disability Concerns. Documentation of the disability or medical concern must be sent to Disability Concerns for verification of disability.Once Disability Concerns has considered the request based on the documentation submitted, the student will be contacted concerning the accommodations request and appropriate housing placement.

Accommodations within Campus Dining Services
Accommodations are available for students who have disability related dietary needs. Requests must be made through Disability Concerns and meet the documentation requirements as verification of needs. All accommodations must eb approved through the Case Cordinator in Disability Concerns. The Case Cordinator will then work with Campus Dining Services to implement the appropriate accommodation.
